Job Position: RN- Case Manager Location: 3555 Cesar Chavez, San Francisco, CA 94110 Start Date: 03/23/2026 Duration: 15 Weeks with possible extension Schedule: 5x8 Days (Fri–Mon rotation) Position Overview The RN Case Manager provides care coordination, discharge planning, and utilization review throughout the acute care stay.
