03-10-2026 End Date: 06-09-2026 Contract Type: Travel Contract
Shift Type: Monday-Friday, 7:30 AM - 4:00 PM (Day shift, 5x8).
Actual start/end may vary; possible schedules include 7:00 AM -
3:30 PM, 7:30 AM - 4:00 PM, 8:00 AM - 4:30 PM, or 8:30 AM - 5:00
PM. Weekends: every other weekend (or 4 weekend days in a 4-week
schedule). Hours per Week: 40 Candidate Pay Rate: $63 per hour

Coordinate timely discharge planning and transitions of care to
improve patient outcomes Perform utilization review and case
management activities using Milliman guidelines where applicable
Develop and document individualized plans of care and needs
assessments, including ordering DME when required Chart accurately
and completely in Epic to maintain regulatory compliance and smooth
handoffs Collaborate with interdisciplinary teams (therapy,
pharmacy, social services, etc.) to remove barriers to safe
discharge Educate patients and families about care plans,
post-discharge resources, and follow-up needs Ensure documentation
and processes meet CMS and HIPAA requirements Float within case
management services as needed to cover gaps and support the unit
Qualifications Active Alaska RN license in hand required for
auto-offer (screen for pending license — AK license required) BLS
certification required ACM (Case Management) certification
preferred Epic experience required (experience with EMR hardstop
preferred) Acute care hospital experience required; behavioral
health experience does not apply Minimum 1 year of RN experience
(first-time travelers accepted) Must be located at least 50 miles
from the facility (locals not accepted) Education Active RN
licensure (AK) required Knowledge & Skills Case management and
utilization review best practices Discharge planning and care
coordination Milliman Guidelines familiarity Proficient Epic
charting and workflow navigation Comfortable using Microsoft Office
and Teams Strong interdisciplinary communication and patient
education skills Understanding of CMS regulations and HIPAA
compliance Experience across patient age groups from neonates to
