Job Summary
The Center Director for the AgeWell PACE (Program of All-Inclusive Care for the Elderly) program has the responsibility for a wide range of disciplines and services provided through the PACE Center, including all administrative responsibility related to direct operation of the PACE Center. The Center Director coordinates overall operations of the PACE Center and serves as the liaison between direct care staff and administration.Position Responsibilities
Coordinates all center activities including care planning and implementation, preparation for interdisciplinary conference and daily meetings, family conferences and initial and period assessments.
Supervisory responsibility for RNs, rehabilitation therapists, clerical staff, social workers, transportation staff, recreational staff, dietary staff, and center aides.
Facilitates and leads morning meetings of the interdisciplinary team (IDT); facilitates and leads regularly scheduled Intake and Assessment meetings to review participant cases on a regular basis.
Oversees the quality of care and safety of PACE participants, both at the PACE center and in the community.
Serves as a liaison between disciplines and between administration and direct staff.
Works closely with the medical care team as part of the IDT to develop and implement participants' individualized care plans.
Coordinates 24- hour care delivery.
Coordinates with the marketing department to hold "friendly visits" for new candidates at the PACE center.
Serves as a representative of AgeWell PACE to the community.
Coordinates activities related to new participants.
Manages the center's budget; communicates regularly with AgeWell PACE administration on center operations and needs.
Oversees overall documentation cohesiveness and accuracy in each participant's medical record.
Other projects and duties as assigned.Possess the Ability To

Troubleshoot efficiently to maintain quality operation of the PACE center.
Be organized within busy center and clinic environments.
Establish and maintain effective interpersonal relationships with all levels of staff, other departments, agencies, members, providers and the general public.
Communicate effectively, both verbally and in writing, with individuals from varying cultural backgrounds.
Prepare concise and clear reports.
Identify pertinent policy and operational issues; develop strategies, solutions and recommendations.
Multi-task on projects and their respective activities, timelines and issues.
Effectively utilize computer and software tools.
Only act within the scope of their authority to practice.
Work in a dynamic and fast-changing environment.
Foster and model an effective team environment, where each team member is held accountable for their independent work.
Approach leadership with an openness to new ideas, cultural humility and acknowledgment of the dynamics of privilege.
Reinforce strategic DEI initiatives set by the leadership team and Board of Directors, and assist in building a culture that models inclusive behaviors, proactively manages bias and provides opportunities for strengthening DEI acumen through development and training.
Meet a standardized set of competencies for the specific position description established by AgeWell PACE and approved by regulatory agencies before working independently.
Be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contact.
Pass a criminal background which will verify that the candidate is: not excluded from participation in Medicare and Medicaid programs; has not been convicted of criminal offenses related to their involvement in Medicaid, Medicare, other health insurance or health care programs, or social services programs under title XX of the Act; and have not been convicted of physical, sexual, drug or alcohol abuse.Knowledge of
Physical, mental and social needs of frail older adults.
Principles and practices of health care, health care systems, and managed care.
Community-based organizations, agencies, and community services.
Culture and needs of the socially and ethnically diverse population AgeWell PACE serves.
Team and value-based care.
Regulatory policies, rules and regulations.
Familiarity and/or experience with geriatrics.Experience & Education
Bachelor's degree in a health-related field required, Master's preferred.
At least 3 years' experience in a supervisory capacity within a healthcare setting.
Either have one year of experience working with a frail or elderly population or, in the absence of such experience, to receive appropriate training from the AgeWell PACE on working with a frail or elderly population upon hiring.
Candidate must be comfortable working in a dynamic environment and comfortable working with both direct care staff and administration staff.
Experience in successfully managing and completing projects is required.
Experience with senior care clinical programs similar to PACE preferred.
Must excel in organizational and supervisory skills, with an ability to effectively handle difficult situations.
Experience working with clinical charts and records.Physical Demands and Work Environment

Employee will also be actively present throughout the PACE center, engaged with employees, participants and with staff. Employee is required to use hands and fingers, especially for typing on the computer and using the mouse. Employee must be able to talk and hear, particularly for daily interactions with staff and elderly participants.
